REGISTRATIONS

STEP 1. Check if you live within the practice boundary?

Please be aware that NHS doctors in the UK are required to visit their patients at home if a patient cannot come to the surgery, therefore we only register patients living reasonably close to the practice. We are based in Bayswater but our core catchment area includes  Westbourne Park, Maida Hill, Paddington, and Notting Hill.


STEP 2. If the answer is “YES” you can register online using our online form.

At the end of the form you will be asked to upload a Photo ID (e.g. passport, driving licence or national ID card) and a Proof of address (e.g. bank statement, utility bill or tenancy agreement). This is not mandatory, but it will help us create a correct and accurate patient record.

For children, please note that we do require a copy of the Redbook (immunisation pages).

We will process all registrations within 14 working days. We will contact you as soon as the registration is completed or if we need some further information.

STEP 2. If the answer is “YES” you can register online using our online form.

At the end of the form you will be asked to upload your  Photo ID (e.g. passport, driving licence or national ID card) and a Proof of address (e.g. bank statement, utility bill or tenancy agreement) not older than 3 months. Please read carefully the instructions and upload those two documents in order to avoid unnecessary rejections.

To register a child under 5 years of age please upload a Birth Certificate and Redbook (immunisation pages) if they are not empty. If the child is above 5 years of age please upload a photo ID with immunisation pages.

We will process all registrations within  14 working days. Please do not send us emails asking if registration is already completed as we are currently dealing with a high volume of tasks and this is placing a strain on providing assistance where is most needed. We will contact you as soon as registration will be completed or we will need some further information.


You can also collect a registration form from reception.

Please download a Registration Form and New Patient Information Questionnaire and print, complete and bring these to the Practice along with your ID.

Alternatively, ask at Reception for the necessary registration forms to be completed. Proof of identity and residency are required. From that point, you will be able to make an appointment to be seen by a Doctor or a Nurse and your notes will be requested from your previous GP.
